Total Health Roadmap

Health care extends beyond medical care. Physical, psychological and social conditions are part of every patient’s total health. Total Health Roadmap is a program through CHI Saint Joseph Health Foundations that connects patients to community resources.

Food availability, safe housing, quality childcare, transportation, access to behavioral health services and social support—these are human needs. When we meet people where they are, it’s more than outreach; we are facilitating a necessary component of health care.

Our grants team works hard year-round to make sure the CHI Saint Joseph Health Foundations continue to support the Total Health Roadmap program.
